    Included

    • Operator certification record
    • Hands-on evaluation form
    • Daily inspection checklist

    Operator training & certification record

The written record OSHA asks for first: operator name, training date, evaluation date, and who did the evaluating.

    Practical (hands-on) evaluation form

A supervisor-friendly checklist for the on-site driving evaluation, with a pass/retrain decision at the end.

    Daily pre-shift inspection checklist

One-page checklist for the start-of-shift inspection OSHA requires before a truck is placed in service.

What OSHA requires you to keep on file

Two records matter most. First, 29 CFR 1910.178(l)(6) requires a written certification for every operator listing their name, the training date, the evaluation date, and the identity of the person who evaluated them. Second, 29 CFR 1910.178(q)(7) requires each truck to be examined before the shift it is used on, and most employers document that with a daily checklist. The forms above cover both.

The part forms can't do for you

A certificate on its own is not compliance. The hands-on evaluation has to happen at your workplace, on the type of truck the operator will actually run, and be signed off by someone qualified to judge competence. That's why our course pairs the self-paced online classroom portion with an employer evaluation kit — the classroom piece is done for you, and the driving check stays where OSHA puts it: on site.

Evaluations expire three years after they're performed. Check your expiry dates with the free renewal reminder tool.
