Updated August 2026 — reviewed against current OSHA 29 CFR 1910.178 requirements
Accepted by employers in all 50 states · 3-year certificate validity
Step 1 — Complete OSHA-aligned formal instruction
Enroll in an online forklift certification course that covers OSHA 1910.178(l)(3) topics: stability, load handling, pre-shift inspection, refueling, pedestrian safety, surface conditions, and hazardous locations. Complete narrated lessons, activities and knowledge checks, then pass the separate final exam.
Start OSHA Operator's online course — instant access, $69, works on phone, tablet, or desktop.
Step 2 — Get practical hands-on training on the truck
Your employer (or a designated qualified evaluator) demonstrates the truck, then watches you perform representative tasks: pre-shift inspection, mounting/dismounting, driving forward and reverse, turning in aisles, picking and stacking loads, refueling. OSHA requires practical training on the specific type of truck you'll operate.
Step 3 — Pass the workplace evaluation
The qualified evaluator confirms competent performance and signs the evaluation form. OSHA Operator supplies the pre-filled form with your name and course details — the employer just checks off performance items and signs.
Step 4 — Employer files the certification record
Per 29 CFR 1910.178(l)(6), the employer keeps a written record with your name, training date, evaluation date, and the trainer/evaluator's name. We provide the template.
Step 5 — Drive, then recertify every 3 years
You're now an authorized operator for the truck class(es) you were trained on. OSHA requires re-evaluation at least every 3 years, sooner after an incident, unsafe observation, new truck type, or workplace change.
