When renewal is required sooner than three years

Three years is the outside limit, not the only trigger. Under 29 CFR 1910.178(l)(4)(ii), refresher training and a new evaluation are required whenever an operator is involved in an accident or near miss, is observed operating unsafely, is assigned a different type of truck, or the conditions in your workplace change in a way that affects safe operation.

What OSHA wants to see in your records

The written certification record must show the operator's name, the date of training, the date of the evaluation, and the identity of the person who did the evaluating. Keeping the expiry date alongside it is what turns a filing cabinet into a system that actually prevents a lapse.

    Frequently asked questions

Three years from the date of the operator's last evaluation, under 29 CFR 1910.178(l)(4)(iii). Refresher training is also required sooner after an accident, a near miss, unsafe operation, a change of equipment type, or a change in workplace conditions.

The operator is no longer certified to operate and must stop until retrained and re-evaluated. An expired certification found during an OSHA inspection is cited the same as no training at all.
