What documentation does OSHA want for forklift training?
Updated August 2026 — reviewed against current OSHA 29 CFR 1910.178 requirements
Accepted by employers in all 50 states · 3-year certificate validity
The minimum record, field by field
- Operator name.
- Date the training was completed.
- Date the performance evaluation was completed.
- Name of the trainer and of the evaluator.
- Recommended: equipment class, topic list, and the operator's signature.
The three gaps that cause citations
- A course certificate with no accompanying hands-on evaluation record.
- A sign-in sheet with no evaluator name or equipment class.
- Records that cannot be produced quickly during a walkaround.
Where to keep it
Keep the record accessible at the site where the operator works, not only at corporate. Employers using the team dashboard can pull a roster and per-operator PDFs on a phone during an inspection.
Frequently asked questions
- How long do I keep the records?
- At least through the current three-year cycle; most employers keep them for the duration of employment.
- Does digital count?
- Yes, as long as you can produce it on request.
- Do I need a separate record per truck class?
- You need an evaluation per class. Many employers use one form with the class recorded on it.
